1 on 1 personal training/small group personal training/Remote Training

DESCRIPTION OF SERVICES:

Fitness Coaching subscriptions include the following:

-Fitness Program co-creation session: We sit down together and you get to make the final decisions on every element of your program so that you always enjoy every workout and get the freedom to achieve your goals the way you want.

-Then I’ll design your program and individualize it from the data gathered from your strength/performance test, make sure it progresses safely/effectively, tracks the objective/subjective metrics that show you how much progress you’re making.

-Each workout has a 2 question HRV survey or 3 question survey asking you to rank your current energy, motivation, and muscle soreness. Then it will recommend the best way to approach your workout that day so that you signal your body to make progress without overdoing it/injuring yourself.

-It will include exercise coaching videos that thoroughly demonstrate how to set it up, and perform the exercise safely and effectively

-The benefits tab will educate you on all the cellular and physiological upgrades your program is doing for you to boost your motivation

-Conveniently access your program anywhere in the world from any device.

-I show up on time, full of energy, and excited to coach you safely/effectively through each workout in person/google meet from your individualized program so that you get all the benefits from the training methods and make progress towards your goals

-If you ever want to do to aerobic conditioning or corrective exercise workouts on other days per week to get the unique benefits that only can be acquired from those training methods, all you have to do is sign up for their baseline testing and I’ll design you an individualized program at no additional cost.

-8 or 12 sessions per 30 day billing cycle that corresponds to your coaching subscription that you selected

-Full gym access at no additional cost on the days/times that we agree upon

-Program design at no additional cost for 1 on 1 and small group coaching subscriptions

-Daily support between 7am-4pm via phone or text if you ever have any questions, or want clarification that you’re doing an exercise right. Otherwise you can always email me

Whether client accessed programs, scheduled/attended their coaching sessions during the paid period or not is immaterial to this agreement. By signing this document, member is agreeing Life Strength Fitness shall not be held liable for member’s use or lack thereof of the service for which the member has purchased.

Payment Expectations:

Clients are required to pay in advance for their subscription. Clients will provide Life Strength Fitness with a valid credit card/debit card to keep on file. Your card will automatically get charged on the same numerical date every month. The recurring date can be changed upon your request if vacation, work travel, medical event, or unforeseen financial struggles arise.

Insufficient Funds:

If client’s on file credit/debit card or bank account(electronic funds transfer) issues Life Strength Fitness LLC an insufficient funds notice or fee, the first incident shall be waived along with a notice of incident from Life Strength Fitness. For any subsequent occurrences the member shall be charged a $20 fee to cover all financial services fees and labor necessary to rectify the insufficient funds situation.

Any client who’s have had more than one(1) insufficient funds notification will be required to either submit an active and valid major credit card(if this has not already occurred), or pay for a full year, in advance.

If you have insufficient funds or an invalid credit card on file you will lose access to your programs until you’ve updated and paid the balance on your account.

Scheduling & Canceling:

Life Strength Fitness LLC and its calendar system requires a 24-hour sign-up window before every 1 on 1 personal training session, small group personal training session, baseline strength/performance testing, aerobic fitness test, and functional movement test. Members shall be able to sign up using the appropriate link on the website at www.lifestrengthfit.com

You will receive SMS and email reminders 48 hours and 24 hours in advance.

Should client be signed up and know that they will be unable to attend the said scheduled appointment, please cancel/reschedule your session 12+ hours in advance using the confirmation email. If you can’t locate your confirmation email text Andrew and he will take care of this for you. Should the member fail to cancel within 12 hours of the appointment and/or not show up to a scheduled appointment, the member will be “checked in” as attending the appointment.

Should the member wish to schedule another test or training session, they will have to purchase another session at full price.

If you’d like to train additional times in a given week, additional 1 on 1 and small group personal training sessions can be purchased at the same price per session as your subscription.

1 on 1 Personal Training Sessions roll over into the next month if they don’t all get used and expire after 3 months.

Small group personal training sessions don’t roll over, so if you don’t use all of your sessions in your 1 month subscription cycle they will be forfeited.

Access to Programs:

A Fitness Coaching Subscription is required for the reservation/access to services and programs of Life Strength Fitness LLC and its affiliates. Whether client accessed their programs and/or attended training sessions during the paid period or not is immaterial to this agreement. Client agrees that Life Strength Fitness shall not be held liable for member’s use or lack thereof of the service for which the member has purchased.

If you have insufficient funds, an invalid credit card on file, put your account on a paid hold, or no longer have a Fitness Subscription, you will lose access to your programs within 24 hours of one of the previous listed events. You will have to choose a fitness coaching subscription and pay off the balance on your account to gain access to your programs again.

Remote training Custom Program Design:

After we sit down and co-create your program, you’ll get an invoice to pay for the custom program design of your first program, which takes a few days to 1 week to complete. Once it is complete I’ll shift the renewal date of your subscription so you get a full 4 weeks of my support and aren’t missing out on coaching support when you don’t have a program yet. All custom program design work needs to be paid up front prior to the start of work. All new programs that get designed fo you in the future are an additional cost when they’re needed.

Cancellation:

Clients that decide to cancel their training subscription must submit the cancellation in writing to andrew@lifestrengthfit.com 2 weeks or more in advance prior to the renewal date of their subscription. If submitted in less than 2 weeks from the renewal period you will be charged at your scheduled renewal date as a final payment for coaching services and will have the opportunity to be coached and benefit as usual for the next 30 days.

Referrals:

Clients who refer others to Life Strength Fitness that sign up for small group training or online training will receive $25 cash as a thank you.

Fat Loss Fundamentals Coaching Program

Description of Services:

Health and lifestyle coaching is a client-centered collaborative and supportive profession that helps empower you to achieve your goals and maintain long term optimal health. This happens through self-discovery of what you already know that can be used to create impactful action steps, self-monitor your lifestyle behaviors and progress to increase your accountability. Health and lifestyle coaches support their clients by helping them set meaningful goals, create action steps, plan for/troubleshoot obstacles, acquire goal-specific knowledge, lifestyle skills, and external resources that will help them.

  • Start with a body fat test (Measure your body weight, surface body fat % with body fat calipers in 7 specific locations telling you how many pounds of muscle and fat you have, and your body circumferences in 11 locations)

    Then I'll analyze your data and you'll receive a report with the following:

    • All of your circumference measurements broken down by each body part, along with how much fat you have in each location

    • What your body fat % is, muscle weight, fat weight, circumferences, and how much improvement you've made from last time (unless it is your first time)

    • What your waist to height ratio is, what does that tell you about your current metabolic health, and the optimal range for you (one indicator of your metabolic health status)

  • Follow up 30 minute client-centered goal setting session where you’ll set 1 SMART outcome goal that is specific, attainable, and meaningful to you with an attached accountability plan which boosts your motivation and confidence.

    Added Bonus: You’ll get to select 3 research supported educational lessons, lifestyle skills, or resources from my library (that currently consists of 70+ easy to understand documents), that you get to keep forever increasing their capability and belief that they can achieve their goal.

  • The 28 day body transformation course will kickoff immediately following that session or on the precise day that’s convenient for you 

  • Every week you’ll acquire more knowledge, lifestyle skills, and additional resources that will boost your competence and confidence that you can achieve your goal. (Everything you get in the course is listed at https://www.lifestrengthfit.com/28daytransformation)

  • Once per week 30 minute lifestyle coaching session where we will celebrate all of your wins, troubleshoot any obstacles you encountered last week, refine/upgrade any of your current action steps, then you’ll set the focus of whatever area of your lifestyle you’re considering making a change in and we will co-create a new action step that you’re excited to do with a self-accountability plan

  • Daily support from a national board certified health & wellness coach for those real world, on the go questions or freakout moments that you encounter. Text me anytime between 7am-4pm or email 24/7 and I'll promptly get back to you 

  • After you complete the course you’ll do another body fat test to quantify your progress and get to select 3 more research supported educational lessons, lifestyle skills, or resources from my library that you get to keep forever.

  • By the end of this course you’ll have made fantastic progress, maybe even already achieved your goal. You’ll be in the habit of creating and successfully executing new lifestyle behaviors that serve you, have a deeper knowledge on relevant topics that will help you, new lifestyle skills, and additional resources that will empower you to keep making progress on your own. 

    If you don’t feel 100% confident that you can do this on your own, you’ll have the opportunity to enroll in a customized lifestyle coaching partnership on a month to month basis until you are 100% confident that you can do this on your own. 

Client Expectations:

The degree at which you participate in lifestyle coaching is your responsibility. I am not liable for how much effort you put into our coaching sessions, your action steps, or the degree of results that you get.

-Willing to commit to creating action steps that will challenge you and help you make progress towards your goals

-Willing to work hard at your action steps that you create, delay short-term gratification, and stay focused on your long-term goals

-Willing to acquire new knowledge, lifestyle skills, progress trackers, and other resources that will make you more competent and confident that you can achieve your goal

-Willing to stay engaged in the process, even when struggles and obstacles present themselves

-Willing to work through struggles and obstacles to create solutions to them so they don’t set you back again in the future

-Will submit your weekly lifestyle coaching check in form 24 hours before every session

-Show up on time for your scheduled sessions

-Will submit your lifestyle coaching follow up form after every session

-Willing to keep track of your own action steps and how often you do them

-Willing to evaluate your action steps for effectiveness

Things I will do:

-Ask you thought-provoking and open-ended questions to help you gain clarity and problem solve your way to solutions.

-Use coaching tools such as affirmations, reflective listening, and summarizing to help inspire, create ownership, and motivate change.

-Helps the client create measurable and actionable health goals, and supports you all the way until you feel 100% confident you can do this on your own.

-Point you in the direction of health resources and special organizations applicable to your needs.

-Provide you with emotional support, encouragement, and accountability.

-Act as a supporter and a mediator in your relationship with the medical professionals you work with.

-Provide you with top notch customer service and timely support via texting between 10am and 6pm. Otherwise you can email me outside of those hours if you need anything.

-Acknowledge and celebrate your successes

-Help you troubleshoot any obstacles that derailed you from doing your action steps,

-Help you create/modify any action steps

-Educate you on a condition, symptoms, or treatment plan, if I’m comfortable with it, to help you achieve the outcomes your physician wants.

-Provide you with goal-specific eduction via google docs or email on the day of your choosing

-Supply you with published research to guide you to deciding what you want implement into your action plan

-Educate you on various diets and supplements, highlight their benefits so you can make an informed decision if incorporating them into your lifestyle makes sense and would help you achieve your health goals.

-Educate you on what are healthy sources of fats, protein, vegetables, fruits, dairy, etc.

-Educate you on what foods or ingredients to avoid that are harmful to your health or goals

-Educate you on what to get rid of in your house that isn’t serving you and teach you how to shop for replacements that will serve you

-Educate you on how to dine out for special occasions, on vacations, and on the go that align with your goals

-If you keep or choose to keep a food diary or use a nutritional tracker for accountability and want to share it with me to better serve you, I’d be happy to look at it.

-Provide you with or guide you to finding recipes and show you how to prepare them to improve your health

-Provide you with or guide you to finding recipes and show you how to prepare them to improve your health

-Educate you about certain blood tests so you can know and heal yourself, along with assist you in finding somewhere to book the tests

-Give you known measurement standards of a blood test.

-During each session we will create easy to execute action steps that implements the educational information that are grounded in science for what/when/why you’re going to do something, what you can track to see if it worked, and how long you must do it to experience the results.

-Give you lifelong access to everything in your lifestyle coaching program

Things I will not do:

-Diagnose an illness

-Treat an illness

-Offer medical advice

-Recommend the discontinuance of medical treatments

-Create a customized nutrition/meal plan to help mitigate the symptoms of an illness

-Prescribe diets or supplements to treat the symptoms of medical and clinical conditions.

-Perform the duties of a mental health professional and help you treat, cure, or fix emotional trauma, depression, or anxiety.

-Conduct a laboratory evaluation

Payment Expectations:

Full payment for this course has to be made up front. Accepted payments methods are major credit and debit cards (Visa, MasterCard, American Express, etc.)

Lifestyle coaching

Description of Services:

Health and lifestyle coaching is a client-centered collaborative and supportive profession that helps empower you to achieve your goals and maintain long term optimal health. Every week you'll receive online access to the next week of your program on the routine day of your choosing:

  • 1-3 Educational lessons, lifestyle skills, or additional resources

  • Any progress trackers that will help you stay organized and accountable

You get lifelong access to all of the material to reference and benefit from

Daily unconditional support for those real world, on the go questions or freakout moments that you encounter. Text me anytime between 7am-4pm or email 24/7 and I'll promptly get back to you 

By the end of your customized lifestyle program you will have made fantastic progress or might have already achieved your goal. You’ll be in the habit of creating and successfully executing new lifestyle behaviors that serve you, have acquired all of the knowledge, skills, and additional resources that you need to finish achieving your goal completely on your own. 

For whatever reason if you don’t feel 100% confident that you can do that you’ll have the opportunity to continue our coaching partnership afterwards on a month to month basis until you are 100% confident that you can do this on your own.

Client Expectations:

  • Help you plan for obstacles that will derail you and create solutions to them if they happen

  • Use coaching tools such as affirmations, reflective listening, and summarizing to help inspire, create ownership, and motivate change.

  • Provide you with emotional support, encouragement, and accountability.

DAILY TEXT SUPPORT FROM 7AM-4PM AND 24/7 EMAIL SUPPORT

  • Any health/lifestyle question, freakout moment you have during the day.

  • Review your food diary or nutritional tracker.

  • Help you upgrade/modify a recipe to help you achieve your goals and avoid sabotaging you.

  • Review a restaurant menu and give you options on what to order that supports your goals.

  • Review any food online/grocery store and let you know if it is a goal-supporting or sabotaging option.

  • Supply you with published research to help you brainstorm what you want to focus on next.

  • Point you in the direction of special organizations applicable to your needs.

  • Act as a supporter and a mediator in their relationship with a medical professional.

Things I will not do:

-Diagnose an illness

Insufficient Funds:

If your card on file credit/debit card issues Life Strength Fitness LLC an insufficient funds or incorrect card numbers notice. Your subscription and any scheduled appointments will be canceled until the issue is resolved.

Termination of lifestyle coaching:

You have the right to terminate the coaching partnership at any point during the process. This must be done 14 days or more in advance of your next monthly payment to avoid the next payment from processing. Clients that decide to cancel their training subscription must submit the cancellation in writing to andrew@lifestrengthfit.com.

You will still get lifelong access to everything you've received up to that point in your lifestyle program. However, you will forfeit receiving the remainder of the educational materials, lifestyle skills, or resources in your program.

Purchasing of non-coaching products:

If you choose to purchase any product from the store, I do make a small commission on each sale.
